Do Students Access the Health Center?

in one of four ways: the School Nurse may refer, parents/guardians may
refer their child, students may refer themselves, and/or the Primary Care

Payment
for Services
The Health Center will bill insurance providers for the cost of each visit.
Sturdy Memorial Hospital will bill the parent/guardian for the appropriate
co-pay according to individual health insurance plans....continue...
Coordination
of Care
Communication with both parents/guardians and the PCP is a priority of
the Health Center. If the student has a doctor, written communication
to the PCP will occur after being seen at the Health Center....continue...
Enrolling
Students in the Health Center
A parental consent form and student data sheet must be completed and kept
on file. Advanced filing will expedite the registration process and get
the students seen as quickly as possible. Consent forms and student data
sheets are available in the Nurse's Office, or by calling the Health Center
at (508) 222-5150 (Ext.) 212
